Practice services
Our practice provides general practitioner medical consultations to respite and permanent care patients in residential aged care facilities. We provide long and short consultations as well as different types of care (eg. complex and preventative) based on patients’ needs. Patients will receive medical services such as general consultations, contribution to care plans, comprehensive medical assessment, case conferences and residential medication management reviews. Telehealth consultations will be available to patients in residential aged care facilities under our care when our doctors are not available on site during our practice opening hours.

Appointment types
Our general practitioner will generally visit each residential aged care facility on a set day each week. If you wish to have your family attend your consultation with the doctor, please ensure that contact us via email at reception@infinityhealth.au to make an appointment.
Our practice operates primarily on an appointment system. However, allowances have been made to cater for the doctor to consult new respite and permanent care patients as they are admitted to the residential aged care facility on the day. Practice consultation fees
We provide bulk billing to patients with a valid Medicare card for general medical consultations.
We charge a small, out of pocket fee for services not covered by Medicare, such as paperwork and forms for taxi cards, disabled parking, capacity/proof of life letters, Centrelink forms, proof of identity for banking services, carer’s leave medical certificates etc. To make a request, please contact our administrative team via email and they will get in touch with you to explain the fee involved and help you arrange payments via bank transfer to ensure your request is progressed without delay.
After hours care
We have a partnership with the National Home Doctor Services for after hours care. This service is available outside the normal clinic opening hours, and nursing staff at the residential aged care facility will be able to contact this service if required.
Home visits
Please contact the nursing staff at the residential aged care facility if you are visiting family on day leave and feel unwell. Our doctors generally are unable to provide home visits outside of the residential aged care facilities, but may be able to provide a telehealth consultation if referred by the nursing staff at the residential aged care facility.
Telephone and electronic communications
If you feel you need to contact our GP, please email the clinic at reception@infinityhealth.au, leave a detailed message and our staff will forward this information to the doctor, who will return your call at their earliest convenience. Secure management of your personal health information and privacy policy
Infinity Health maintains a fully computerised medical record system. Your medical record is a confidential document. It is our firm policy to maintain privacy and security of personal health information at all times, and to ensure that this information is available only to authorized members of our staff. No information will be conveyed to any third party without your express prior consent. We comply with the Privacy Act (1988), the Health Records Act 2001 (Vic), the Health Records Regulations 2012 (Vic) and the Privacy and Data Protection Act 2014 (Vic).
